How to Obtain a Certificate:

The cost for a certificate is $17.00, and the cost of a second copy is $10.00 per order.

  Requires MasterCard or Visa. Please note, all online orders includes an additional processing fee.

  • Order a Birth Certificate Online (secure)
  • Order a Death Certificate Online (secure)

 

• You may mail your order:
• Print mail-in Birth Certificate Application*
• Print mail-in Death Certificate Application*

Please note that a Photo ID must accompany all orders.

Be sure to enclose the appropriate fee(s) along with a check or money order payable to Vital Records.

Please enclose a self-addressed stamped envelope if possible; it will help avoid delays.
