What is Healthy Children and Families (HCF)?
The goal of Healthy Children and Families (HCF) is to improve the health, safety, and well-being of children participating in programs provided by the Department of Human Services (DHS).
HCF works closely with DHS to provide case consultation to case workers and home visiting services to families. Through home visiting, HCF staff provides families with the knowledge and resources to thrive. This includes connecting families with existing community resources, providing health education and giving physical and mental health assessments. Home visits are conducted by a licensed nurse and a community health educator.
Who does HFC serve?
Families with infants and young children participating in DHS programs who have medical or safety concerns. Examples of medical and safety concerns include, but are not limited to the following:
- Drug exposure
- Oral health
- Car seat safety
- Safe sleep
- Chronic illness
- Sexual health
- Child development
- Medically fragile
