Please bring the following with you to every visit:

  • Immunization records
  • Photo ID
  • Insurance information

Low-cost vaccines are provided for children (birth through 18 years) who are:

  • Medicaid-eligible (including CHP+)
  • Uninsured
  • Underinsured
  • American Indian or Alaska Native

Bridge Access Program Eligibility (COVID-19 vaccine only): Individuals who are 18 years and older and uninsured or underinsured are eligible to receive a free COVID-19 vaccination.

State Flu Vaccine Eligibility (flu vaccine* only): Individuals who are 19 years and older and are uninsured or underinsured are eligible to receive a free flu vaccination.

Medicaid Eligibility (COVID-19 and flu vaccines*): Individuals who are Medicaid eligible and are 19 years and older are eligible to receive a free COVID-19 and/or flu vaccination.

*Flu vaccines will be available through June 30th (or as long as supplies last)

  • Colorado Immunization Information System (CIIS) is a confidential, population-based, computerized system that collects and consolidates vaccination data for Coloradans of all ages from a variety of sources. CIIS assists healthcare systems, schools, and individuals to keep a record of the immunizations you/your child has received. If you prefer to exclude yours or your child’s information from CIIS, visit the CIIS opt-out procedures web page to opt out of the registry. You will also find information on how to rescind a prior opt out.
