Low-cost vaccines are provided for children (birth through 18 years) who are:
- Medicaid-eligible (including CHP+)
- Uninsured
- Underinsured
- American Indian or Alaska Native
Payment information:
- EPCPH charges fees of up to a $2 minimum or 2.39% of the total, whichever is higher. To avoid these fees, customers may use cash, checks or money orders while in the office. Online payment options may be available and include debit, electronic check and google pay. Fees are listed on the payment page.

CIIS is a confidential, population-based, computerized system that collects and consolidates vaccination data for Coloradans of all ages from a variety of sources. CIIS assists health care systems, schools, and individuals to keep a record of the immunizations you/your child has received. If you prefer to exclude yours or your child’s information from CIIS, visit the CIIS opt-out procedures web page to opt out of the registry. You will also find information on how to rescind a prior opt out.
